Steve Irwin died on September 4, 2006, while filming a documentary called Ocean’s Deadliest off the coast of Queensland, Australia. He was fatally injured by a short-tail stingray during a shallow-water shoot.

The stingray’s barb pierced Irwin’s heart, causing immediate, fatal trauma. According to the official autopsy report, the injury was accidental and occurred when the ray reacted to being boxed in by the crew. Irwin died within minutes.
